The wraparound process aims to achieve positive outcomes by providing structured, creative, and individualized team planning. The team planning process incorporates four specific phases (engagement, plan development, implementation, and transition). This model, compared to traditional treatment planning, results in plans that are more effective and more relevant to the youth and family. Wraparound plans are more holistic than traditional care plans in that they are designed to also meet the identified needs of caregivers and siblings, they also address a range of life areas. Through a team-based planning and implementation process, wraparound services aims to teach/develop problem-solving skills, coping skills, and self-efficacy with the young person and their family members. This model includes an emphasis on integrating the youth into the community by building a social support network for the youth and family.
